Join our team as the Clinical Director, Care in the Home , a senior physician leader responsible for shaping and scaling innovative home and community-based care models. This role provides strategic, clinical, and operational leadership across programs including Hospital at Home, Advanced Care at Home, ED/Urgent Care at Home, Care After Discharge, and services supporting high-risk populations.
Key Responsibilities
- Maintain clinical practice through direct patient care (approximately 0.3 FTE) with prorated participation in weekend and holiday coverage.
- Lead the vision, strategy, and growth of Care in the Home programs in partnership with operational leadership.
- Oversee clinical quality, safety, governance, and performance across home-based care services.
- Drive development of care pathways, readmission reduction initiatives, and post-acute care partnerships.
- Collaborate with hospital, ambulatory, population health, digital, and community-based teams to improve access, outcomes, and patient experience.
- Champion virtual care, remote patient monitoring, and innovative technologies that extend care beyond traditional settings.
- Mentor physician and advanced practice clinician leaders while fostering a culture of excellence, accountability, and innovation.
- Directly supervise three clinician leaders who collectively oversee 12 physicians and APCs.
Qualifications
- Board-certified physician with an unrestricted Delaware medical license and DEA.
- Minimum 10 years of clinical experience in hospital medicine.
- At least 3 years of leadership experience overseeing clinical programs or care delivery operations.
- Experience with telehealth, virtual care, and value-based care models required.
- Advanced management degree or certification (MBA, MPH, MHA, MPA, ACPE) preferred.
$340,829 - $350,829 This pay rate/range represents ChristianaCare's good faith and reasonable estimate of compensation at the time of posting. The actual salary within this range offered to a successful candidate will depend on individual factors including without limitation skills, relevant experience, and qualifications as they relate to specific job requirements.
