RegalCare at Wakefield is currently seeking a licensed Social Worker to take on the newly created Program Director position at our facility. Prior experience working in behavioral health settings are preferred.
Duties
- Participate in development of an individualized care plan with the Social Service Director, Recreation Director, and nurse management team
- Identify and manage barriers to achievement of care plan goals
- Identify and implement effective interventions based on opportunities identified in members plan of care
- Work collaboratively with the Social Service Director, Recreation Director, and nurse management team
