Case Manager - Path Program

Join the Cumberland County Guidance Center as a Full-Time Case Manager with our PATH (Projects for Assistance in Transitioning from Homelessness) Program.
In this rewarding community-based role, you'll help connect individuals to housing, mental health services, healthcare, public benefits, and community resources while serving as a trusted advocate and support system. What You'll Do
As a PATH Case Manager, you'll work directly with individuals experiencing homelessness and serious mental illness to help them obtain stable housing and supportive services.
Key responsibilities include:
- Conducting community outreach and engagement activities
- Developing Individual Recovery Plans (IRPs)
- Providing ongoing case management and care coordination
- Assisting clients with permanent, supportive, and emergency housing resources
- Connecting individuals with mental health, medical, substance use, vocational, and educational services
- Assisting with public benefits and entitlement programs
- Utilizing Motivational Interviewing (MI) and recovery-oriented interventions
- Maintaining accurate clinical documentation and compliance requirements
- Supporting recovery and long-term independence

What You'll Need
Required Qualifications
- Bachelor's Degree in Social Work or a related Human Services field
- Minimum two years of experience in fields such as:
- Case Management
- Behavioral Health
- Mental Health Services
- Social Services
- Homeless Services
- Human Services
- Community Outreach
- Knowledge of housing resources and public benefit programs
- Strong communication, advocacy, and relationship-building skills
- Valid driver's license with acceptable driving record
Preferred Qualifications
- Experience working with individuals experiencing homelessness or housing instability
- Experience supporting individuals with serious mental illness or co-occurring disorders
- Knowledge of Housing First and supportive housing models
- Experience with crisis intervention and care coordination
- Bilingual English/Spanish
- Experience utilizing Motivational Interviewing and trauma-informed care
