Licensed Practical Nurse – Personal Care Weekend Program
The Licensed Practical Nurse – Personal Care Weekend Program provides quality nursing care to each resident in accordance with DPW regulations, while considering each resident's unique needs and their mental and physical abilities. This position will be performed in accordance with TANDEM LIVING Core Values, Mission Statement, and regulations.
Location: Trillium Place
Current Opening: Every Saturday & Sunday — 8-hour shifts
Days: 2:00 PM–10:30 PM
Will work every weekend and every-other-holiday rotation

Qualifications:
- High school diploma or equivalent.
- Current Pennsylvania LPN license to practice as a Licensed Practical Nurse.
- Current hands-on CPR and First Aid certification, or willingness to obtain and maintain.
- Experience in assisted living or healthcare preferred.
- Basic computer skills required. Proficiency with Microsoft Office, including Word and Outlook.
Responsibilities Include:
- Administers and charts medications using the computerized medication delivery system.
- Assesses skin integrity and implements appropriate procedures for skin tears.
- Performs LPN-scope clinical tasks including catheterizations, bladder scans, specialized treatments, and calling the MD for verbal orders.
- Oversees assigned staff and serves as the in-house LPN Supervisor when no Clinical Coordinator is present.
- Completes required documentation such as admissions, discharges, hospital transfers, assessments, support plans, and DME/MA51 forms.
- Monitors and reports resident health changes and responds appropriately to evolving needs.
