The Director of PACE will establish and lead Connecticut's first Program of All-Inclusive Care for the Elderly in rural areas, reporting to the Deputy Director of Rural Health Transformation. This role, part of a $100+ million annual federal Rural Health Transformation Program grant, will oversee a regional PACE hub providing integrated primary, behavioral, and long-term care services to dually eligible older adults. The position focuses on expanding access to community-based services that reduce hospitalizations, support aging in place, and prevent institutional care through an urban-rural care model.
Key qualifications:
- Experience with Program of All-Inclusive Care for the Elderly (PACE) or related health policy in home and community-based services, long-term care, or integrated care models
- Clinical background relevant to PACE, preferably as a registered nurse with health or clinical policy experience
- Programmatic leadership experience designing, implementing, and monitoring large or complex projects
- Experience overseeing project teams across in-house staff, outside contractors, partners, and providers
