Identify and manage operational risks and challenges arising during the transition.
Develop and monitor transition plans, priorities, milestones and deliverables.

Monitor service performance and identify areas requiring intervention or additional support.
Ensure operational issues are identified quickly and addressed effectively.
Provide regular updates to senior leadership on progress, risks, challenges and outcomes.
Ensure the successful transition and integration of the schemes into the organisation's operating model.

Proven experience managing complex change and organisational transitions.
Strong project management skills and the ability to manage multiple workstreams simultaneously.
Experience managing significant operational challenges and competing priorities.
Strong stakeholder management and communication skills.
The ability to work confidently with senior leadership teams and operational colleagues.
A structured, analytical and solutions-focused approach.
The ability to provide calm, visible and credible leadership during periods of change.
A genuine commitment to the principles of independent living and person-centred Extra Care.
